So last night I was in the process of mating my head back to the block. I found this washer in my sandwich bag that I used to store all the hardware for the cam gear and associated hardware, but I have no idea where it came from. Its warped and seems to be that way on purpose. I'm wondering if it came from the balancer or the crankshaft nut and I just happened to put it in with the cam gear for some reason.
(also bonus if you can tell me what those ear shaped washers are too)

The "ear shaped" washers are from the driveshafts.
To identify the other washer would need inner and outer diameter and thickness.

Me thinks you hit the nail on the head regarding that giant Bellville (wavy) washer
Been a long time, but I seem to recall that washer being installed behind the "big honkin' nut" on the crankshaft that holds the harmonic balancer in place, with about a million foot pounds of torque
But like Ron said, the ID measurement will give somebody the ability to match up to the end of the crank

a Belleville washer is strictly speaking a disc spring, i.e. a slightly conical washer. That is really a 'wave washer' although I am sure it has other names too.
The crank hub washer is #6 here

I agree the two-holed washers look like the ones on the driveshaft flanges, but I wonder if there is another place where these are also found?

33211209543 Called a "reinforcement" and goes under the bolts that hold the shafts to the output flanges on the diff
